010 Temporary Medical Transportation Medical Transportation (temporary and long term) February 28th, 2017
Statement

Medical Transportation arrangements may be made if a student meets the following criteria:

  1. The student has a medical condition that: prevents the student from boarding a bus and walking to seat (ie. broken leg or ankle); prevents the student from walking to their designated bus stop; the student has sustained an injury that prevents them from walking to school.
  2. Has a temporary condition requiring medical transportation (in this case, the request must be accompanied by a doctor’s note).
  3. Transportation for short or long term medical conditions that are not directly related to a Board sponsored program/activity must be approved by the school board.
Restriction

Access to Medical Transportation (temporary and long term) services are available provided the student’s primary address is within the school of attendance boundary.

Procedures

The Parent/Guardian and Medical Practitioner Shall:

  1. Complete and sign the Request for Transportation for Medical Reasons (TF011) – Section 1.
  2. A medical practitioner completes a Request for Special Transportation for Medical Reasons (TF011) – Section 2. This needs to clearly outline in specific terms why a student's medical condition necessitates the provision of transportation for the duration specified.
  3. Forward the request to the school.

The Principal Shall:

  1. Provide and ensure the parent/guardian completes and signs Request for Transportation for Medical Reasons form (TF011) – Section 1.
  2. Sign the Request for Transportation for Medical Reasons form (TF011) – Section 1.
  3. Forward the request to STWDSTS.
  4. Once STWDSTS has arranged transportation, the Principal / school designate shall ensure the parent is aware of the transportation details.

STWDSTS shall:

  1. Organize the most cost-effective transportation for approved requests.
  2. Maintain an up-to-date list of students receiving Transportation for Medical Reasons; indicate the type of transportation provided and the date when the approved transportation will end.
  3. Provide parents, schools and Board administrators with updated student transportation information via GeoQuery.
