This form is intended to notify your student(s) school and Wellington-Dufferin Student Transportation Services (STWDSTS) that your student(s), would like to Opt-in for the school year commencing September 2024.

By submitting this form, you the Parent(s) / Guardian(s) acknowledges that the above-named student(s):
- are eligible for school bus transportation according to STWDSTS policy, and
- will be removed from school bus transportation in the directions noted on this form, and,
- understands that this opt-in status will be carried forward yearly and can be reversed at any time upon notification that transportation is required.
